As part of the qualifying requirements for assistance from the Mental Wellness Fund, we require details regarding your financial situation. This information is necessary in determining what level of assistance (if any) you qualify for.

If you would like to apply for assistance, please submit a copy of proof of income such as:

• your most recent tax return
• social assistance voucher
• any other proof of income

Your eligibility for assistance will be determined upon review of all information submitted as well as any information we may already have on file.

You will be notified in writing of the outcome of our decision. Please note that all conversations and personal information received will be held in strict confidence.
